What Drives Us
Founded in 2016, Ties to Tomorrow Care Management was inspired by the vision of helping individuals transition from dependency to independence. Our name reflects our mission: to forge strong connections between the support we provide today and the successful, independent tomorrows of the individuals we serve.
Support Services
Support Coordination at Ties to Tomorrow involves developing, identifying, coordinating, monitoring, and maintaining supports and services for individuals with intellectual disabilities. Our approach is tailored to each individual, focusing on building a comprehensive plan that addresses both immediate needs and long-term goals.
Our Process
From initial assessments to the ongoing management of services, our support coordinators act as dedicated advocates for our families. Support coordinators help set up services through the Division of Developmental Disabilities (DDD), manage annual budgets, and ensure that all services meet our high standards of quality and effectiveness.
