To request services, individuals begin by connecting with their assigned Support Coordinator. Together, they'll review the person’s service plan, goals, and needs. From there, our DDD Program Director reaches out to discuss how we can best support them, followed by an introductory meeting with a Direct Support Professional. Once that meeting is successful, the Support Coordinator will authorize services to begin.
The Division of Developmental Disabilities (DDD) is a New Jersey state agency under the Department of Human Services. It provides critical supports and services for individuals with developmental disabilities, empowering them to live as independently as possible. These services include support coordination, residential programs, day programs, employment services, and more. Individuals must meet eligibility requirements and be enrolled in the Medicaid-based Supports Program or Community Care Program to access services.
Ties to Tomorrow’s Support Coordinators are highly trained professionals with expertise in social work, psychology, and disability services. Many hold certifications such as CPR and AED and undergo specialized training. More than case managers, they serve as advocates—dedicated to helping each individual thrive through compassionate, personalized care.
