Support Coordination helps individuals with developmental disabilities access services and supports based on their needs and goals. A Support Coordinator works with you and your family to create and manage an Individualized Service Plan (ISP), connecting you to resources like day programs, job training, residential services, and more. 

To qualify, individuals must meet the eligibility criteria set by the Division of Developmental Disabilities (DDD) and be enrolled in either the Supports Program or Community Care Program through Medicaid.

You’ll need to complete the DDD Support Coordination Agency Change Form, which can be found on our "What We Do" website page. This form must be submitted to DDD for processing. The agency change typically becomes effective at the beginning of the following month.

Yes, there is a section on both the Selection and Change forms where you can list the name of a preferred Support Coordinator. While requests are considered, placement depends on availability.
